Do you have any of the qualifying medical conditions listed below:

  • Lupus
  • Intractable Pain (conditions of chronic pain include, but are not limited to, Arthritis, Fibromyalgia, Migraines, Spinal and Muscular Issues, etc.)
  • Post-Traumatic Stress Disorder
  • Inflammatory Bowel Disease (includes Crohn’s Disease, IBS, UC, etc.)
  • Cancer
  • Seizure Disorders
  • Peripheral Neuropathy
  • Multiple Sclerosis
  • Autism Spectrum Disorder
  • Sickle Cell Disease
  • Amyotrophic Lateral Sclerosis
  • Parkinson’s Disease
  • Alzheimer’s Disease
  • Patient is in Hospice Program, either as inpatient or outpatient
  • Tourette’s Syndrome
  • Epidermolysis Bullosa
  • Mitochondrial Disease
  • HIV Stage III

You must provide medical documentation to verify your qualifying condition. It can be obtained from your medical records, a letter from the doctor treating you, or a visit summary from a patient portal.
