To make sure you qualify for certification, please answer the following questions and enter the required information.

If you have more than one qualifying medical condition, you only need to provide documentation for one condition. Once you qualify for the Card based on one condition, we can advise you on which cannabis products would be best to address each and all of your conditions. So, whichever of the conditions that you would be able to get documentation on the easiest or quickest, that is the one you should pursue.
The medical documentation you provide to verify your qualifying condition can be obtained from any of the following sources:

A) Your medical records from the doctor treating you for your qualifying medical condition as long as they show the diagnosis and treatment plan.

B) You can obtain a letter from the doctor verifying the diagnosis of your qualifying medical condition and treatment plan.

C) Visit summaries and medical records can be obtained from a patient portal to verify your condition. It should show your treatment plan and it would be helpful to include any relevant diagnostic tests such as MRIs, bloodwork, etc.



If you answer yes but do not qualify for certification or need clarification, someone from our medical team will reach out to you through email or text.
If you qualify for certification but do not wish to make an an appointment at this time, please come back when you are ready to proceed. We would appreciate the opportunity to certify you and help you find the best Low-THC products for your needs.
