In order to make a Card Renewal appointment, you need to have been certified by Dr. Madelaine Murad, Medical Marijuana Therapeutics, AND your Card must be within 2 months of expiring. If both of these requirements are met, please answer the following questions and enter the required information.

If you answer yes but do not qualify for our Card Renewal Services, someone from our medical team will reach out to you through email or text with further explanation.
If you qualify for Card Renewal Services, but do not wish to make an appointment at this time. please come back when you are ready to proceed.
