To make sure you qualify for the Non-Certification Package, you should
have been certified by a doctor other than Dr. Madelaine Murad of Medical Marijuana Therapeutics and should have a current Georgia Low-THC Oil Registration Card. If so, please answer the following questions and enter the requested information.
